Afterschool Programs for Winter 2008

PATCH 21st CCLC would like to invite the students of Spencer Middle School to attend the Spring Session of programs. Please fill in the following information completely – FRONT AND BACK - and return this form to your child’s school or homeroom teacher. Most classes are filled in the order that the forms are sent back, so get your form back as soon as possible so that your child will have a better chance of getting the class he/she desires to participate in.

Classes will begin the week of January 28, 2008.

Mark beside each program (per day) your child would like to attend by choice preference #1, #2, #3 or #4. Notification will be sent to your child to let you know which programs your child is registered to attend. Students may attend only one class per day.

Mondays:
_____ Sign Language with Bobbie Jo Weygand: Students will learn American Sign Language.
_____ Cooking with Angie West: Students will learn how to cook nutritious meals at the school kitchen.
_____ Fly Tying/Fishing with Zach Zdanek: (Held at COA) Learn to fly fish and tie your own flies!
_____ 5th-6th Grade Skateboarding with Marshall Jarvis: (Held at COA) Learn Skateboarding skills.
_____ Choir with Bridgett Bowen: Students will learn songs for a performance at the end of the session.

Tuesdays:
_____ Ham Radio with R. Moore: Students will learn about Ham (amateur) radio.
_____ 7th and 8th Grade Skateboarding with Marshall Jarvis: (held at COA) Learn Skateboard skills and video.
_____ Basic Cake Decorating with Diana Brown: For beginning cake decorating students.
_____ Make and Take arts and crafts with N. Moore: Students will work on a different craft projects they can take home each week.

Wednesdays:
_____ Drawing Cartoon Characters with Mike Randolph: learn to draw cartoon and comic book characters.
_____ Glass Fusing with Teresa Holcomb (at COA): Students will learn to make glass jewelry and other glass art.(only 1st 6 students get in)
_____ Guitar/Recording with Cam Wilson (at COA): Students will learn about playing guitar and recording music.
_____ Service-Learning/Advocacy with Marshall Jarvis (at COA): Students will develop and work on Service-Learning Projects as well as learn
about promoting and advocating for skateboarding.
_____ Scrapbooking with Tammy Michael (at COA): Student will design and create scrapbooks.

Thursdays:
_____
_____ Hands-On Science with Mrs. Ellison: ( 5-8) Students will be participating in FUN science activities.
_____ Advanced Cake Decorating with Diane Brown: This class if for students who have had cake decorating before and want to learn more.
_____ Fitness Center Program held at RGH Fitness Center: Students will go to Roane General Hospital Fitness Center for an exercise program.
PARENTS MUST SIGN BELOW TO GIVE US PERMISSION TO TRANSPORT STUDENTS TO THE FITNESS CENTER.

Tues. and Thursday:
_____ Drama with Susanna Wilson: Students will learn theatrical skills for plays along with character education.

All SMS PATCH Programs for Winter will begin the week of January 28 and run for approximately 8 weeks.

? Parent/Guardian must pick up students promptly at 5:00 p.m. after each program.
? YOU MUST SIGN THE BACK OF THIS FORM IN ORDER FOR YOUR CHILD TO BE ABLE TO ATTEND PATCH PROGRAMS.


PATCH 21ST CCLC has permission to transport my child to the Fitness Center for the Fitness Programs. I understand that I will be responsible for picking up my child at the Fitness Center at 5:00 p.m. on Thursdays.
